Terms & Conditions - ‘Kogan $50 Voucher Promotion’

1. These Terms & Conditions apply to the ‘Kogan $50 Voucher Promotion’ offered by Choosewell Health Insurance Pty Ltd, ABN 17 234 386 561 (“Choosewell”) (“the Promotion”).

2. We recommend that you read these Terms & Conditions in full. Participation in this Promotion is deemed acceptance of these Terms & Conditions. If you have any queries in relation to these Terms & Conditions, please contact ‘Choosewell Customer Service’ on 1300 421 154.

3. If you are an Eligible Person (see sections 5 and 6 below) then, subject to these Terms & Conditions, you will be entitled to a Kogan Gift Card up to the value described in section 8 below.

4. The “Promotion Period” for the purposes of these Terms & Conditions and this Promotion starts at 10:01am AEST on 14 October 2015 and ends at 11:59pm on 30 November 2015.

5. “Eligible Persons” are defined as persons who:
a. are Australian residents aged 18 years at the start of the Promotion Period;

b. are not current employees or consultants of Choosewell or any related body corporate of Choosewell (as that term is defined in the Corporations Act 2001 (Cth)) (“Related Body Corporate”);

c. have not received a quote from Choosewell or purchased through Choosewell a “Hospital” or “Hospital and Extras” health insurance policy in the 12 months prior to 14 Oct 2015;

d. provide Choosewell with a correct and current email address and residential address or Post Office Box address (“Address Details”);

e. hold a current ‘Hospital’ or ’Hospital and Extras’ health insurance policy (excluding “Overseas Visitor Cover”, “Extras only”, “Corporate policies” or membership of any restricted fund) (an “Eligible Policy”, with “Eligible Policies” having a corresponding meaning);

f. speak to a Choosewell telephone consultant (via an inbound or outbound call) during the Promotion Period and that Choosewell telephone consultant cannot provide the Eligible Person with a cheaper quote on a comparable level of “Hospital” or “Hospital and Extras” cover to the Eligible Policy that the Eligible Person is currently covered by (including after taking into account any Lifetime Health Cover loading and any applicable Government rebates and the customer’s chosen payment method). Comparable coverage identified by Choosewell for “Hospital and Extras” cover may comprise cover by two separate insurers (as determined by Choosewell); and

g. inform or expressly acknowledge to the relevant Choosewell sales consultant that they wish to claim the benefit of this Promotion.

6. The Promotion is not available to any persons who:
a. receives a quote on a “Hospital” or “Hospital and Extras” policy using the Choosewell website; or

b. are unable to obtain a cheaper quote from a Choosewell telephone consultant but who purchases a “Hospital” or “Hospital and Extras” policy from Choosewell regardless during the Promotion Period.

7. How to claim
a. Eligible Persons who cannot be provided with a cheaper quote (as set out in section 5 above) will be able to participate in the Promotion and will be provided with a unique Choosewell customer quote identification number for use during the Promotion (“Live Submit ID”).

b. Eligible Persons will be sent instructions via email on how to claim the $50 Kogan Voucher.

    • Customers will need to reply to the email with a copy of their most recent ‘membership statement’ from their fund that clearly shows the name of the policy and premium amount. Statement must also show some personal details such as name, address, date of birth.
    • Eligible Persons must claim through this process in order to claim the benefit of this Promotion, with no exceptions.
    • Eligible Persons must also provide supporting documentation proving their identity and fund/policy membership (including the cost and details of that membership). Supporting documentation may be provided in any of the following file formats: jpeg, tiff, png, pdf, bmp or gif. Eligible Persons must use the email generated from their email address to claim, with no exceptions.
    • Emails received by Choosewell from unknown or alternate email addresses will not be accepted.

c. Choosewell will verify the information provided and send a further email to the email address specified in the Address Details to confirm whether their claim has been successful.

d. Eligible Persons who are entitled to claim the benefit of the Promotion must complete and submit the claim form as set out in this section 7b within 5 days of the end of the Promotion Period, with no exceptions.

8. Gift Voucher receipt and usage
a. If their claim for the benefit of this Promotion is successful, Eligible Persons will receive a Kogan eVoucher (“Gift Voucher”) to the value of AUD $50 by email to their nominated Email Address by 30 November 2015 (if claim approved in October 2015) and by 15 December 2015 (if claim approved in November 2015).

b. The Promotion is limited to one Gift Voucher per household and Gift Vouchers are not redeemable for cash or any alternative form of credit.

c. Use of the Gift Voucher must comply with Kogan’s conditions of use.

9. Privacy

10. General
a. The Promotion is not available in conjunction with any other promotion, offer or competition offered by Choosewell.

b. Choosewell reserves the right to disqualify any individual from participating in the Promotion who is involved in any way with interfering or tampering with the Promotion including the claim process or who has acted dishonestly or fraudulently in claiming the Gift Voucher.

c. Choosewell reserves the right to vary the Terms & Conditions of the Promotion and/or to suspend, extend or terminate the Promotion or vary the Promotion at any time. This includes the right to substitute the Gift Voucher with a product of equal or greater value in circumstances where the Gift Voucher becomes unavailable for any reason. Any decision of Choosewell on matters in relation to the Promotion is final and no further correspondence will be entered into.
